Cancer evidence: FDA approves a targeted breast-cancer combination; survival maturity remains limited

FDA approved imlunestrant plus abemaciclib on September 18 for adults with ER-positive, HER2-negative, ESR1-mutated advanced/metastatic breast cancer progressing after endocrine therapy. This is a defined indication, not a cure.
Evidence, status and limits
EMBER-3 was a randomized, open-label, active-controlled multicentre trial of 874 adults. In an exploratory 159-patient ESR1-mutated subgroup, progression-free survival versus imlunestrant alone was 11.1 versus 5.5 months; HR 0.53 (95% CI 0.35–0.80). The median difference is 5.6 months; the hazard reduction is 47%, not 47% more survivors. Overall survival was immature at 35% deaths. Warnings include diarrhea, neutropenia, lung inflammation, liver injury, thrombosis and fetal harm. The FDA notice does not supply full follow-up or independent replication; regulatory review is not peer review. Lilly developed the drugs; FDA approval does not establish MAHA sponsorship of the research.
Why it matters / inference
A new option is available for a biomarker-defined population, with meaningful toxicity and unresolved mature survival benefit. Separately, no new replicated evidence establishes one autism cause: genetics, prenatal/perinatal exposure associations, proposed mechanisms and surveillance changes must remain distinct. Vaccine-autism claims remain unsupported by high-quality causal evidence.
What to watch next
Full trial publication and safety frequencies, mature survival, replication, access and price; independently replicated autism findings.
